Who do you call when the DNR requires that you obtain a crossbow evaluation?

The Aurora Advanced Healthcare team of occupational and physical therapists is committed to providing services that enable all patients to participate in activities that they enjoy, regardless of their level of function. Our highly trained professionals understand the required range of motion, strength and endurance that is necessary to perform numerous activities, including hunting! With a referral from your physician, and an application form from the DNR, you can make an appointment with either an occupational or physical therapist to evaluate whether or not you qualify for a crossbow permit. The form for crossbow evaluation can be obtained from the DNR. You must bring your form along for your appointment and your therapist will then perform a battery of tests required by the DNR. The patient is then responsible for forwarding the results to the DNR.
